APPLICATION PROCESS AND REPORT FORMAT:

 

Application Process:

 

There are two basic steps in the application process:

 

Step one - submit “Intent to Apply” application to let us know you are interested.  This application must be submitted by October 31st, 2008 before you can submit your full application and report. To download the Intent to Apply Application .pdf form click here.  If you do not have Acrobat reader you can download it from the Adobe Web site.

The Intent to Apply application requires a $50 fee. 

  • You can send a check with your application to the ASQ Treasurer c/o Mike Beyer, 7117 Rachaels Run, Hamilton, OH 45011

  • or pay by credit card through PayPal by clicking on this link.

 

Step two – Submit your “Application Report".  Complete a project coversheet and report that describes your quality improvement project.  The full application is due by November 28th, 2008.  To download the Application/Report Template click here.  The report guidelines and format are as follows.

 

Application Report Guidelines:

 

The written report should address each of the 5 judging criteria (see below)

and conform to the following guidelines:

  • 10 page maximum length (not including cover page)

  • Include Project Code issued when Intent to Apply application is received

  • Do not include any identifying information other than project code

  • Print title in bold type

  • Clearly indicate report headings (criteria areas in report format area) in bold type

  • Include page numbers on pages 1 – 10 centered at bottom of page

  • Prepare report in MS Word 2000 - 2003 or compatible format

  • Single side, 81/2 X 11 page with Arial 12 point font

  • Both color and black & white print may be used

  • Graphics, charts and tables are acceptable

  • Keep report size to 3 mb or less

  • Narrative must be in English

  • The application narrative should address the five judging criteria

 

Report Format:

 

The final report should include the following sections:

 

     Header:

  • Code - to be issued by ASQ

  • Title of Project - Use bold type for title

  • Project Summary

      Narrative Addressing Judging Criteria:

  1. Project Selection

  2. Situation Analysis

  3. Solution Development

  4. Project Implementation and Results

  5. Team Management

 JUDGING:

 

Judging Criteria:

 

All Quality Award applications will be evaluated based on the following 5 judging criteria:

 

  1. Project Selection (20 possible points) - Explain the methods used to define the problem and select the project

  2. Situation Analysis & Measurement (20 possible points) - Explain the method(s) used to identify potential causes and how the team selected the primary “root cause”

  3. Solution Development (20 possible points) - Explain the process used to identify potential solutions and how the team selected the final solution to implement.

  4. Project Implementation and Results (20 possible points) - Explain the approach used to implement the solution and the net results achieved.

  5. Team/Project Management (20 possible points) - Explain how the team members were selected and the project was managed.

 

Scoring System:

 

Each section of the report will be scored from 0 to 20 based on how well it addresses the requirements.  The total possible score ranges from 0 to 100.  Each judge will study and score each proposal.  A rating summary sheet and narrative summary will be prepared for each application and exchanged with all judges.  The judges will then meet to discuss the applications and revise their scores if desired.  The applications with the 4 highest scores will be selected as the winners.  If two of the finalists receive the same score the judges will then vote on their rank order.
